Athletic Update

All Student athletes have to make sure that they have had a physical examination to compete in athletics at Westford School. This policy has been in place for the rest of the district for years. All students must have proof of a physical exam form on file With the school Nurse, Deborah Travis. A physical is considered current for two years. Proof is an artifact from the doctor’s office with the date of the exam and it must state that they are fit to participate. If it is easier for you, give the fax number to your doctor and have them send it to the school directly. As of now, students need to have all of their paperwork in order to participate fully. Full Paperwork includes: Consent Form, Medical Form, Uniform Agreement and Proof of Physical Exam. If you have questions please feel free to contact me. Below I have included the actual policy. This policy also includes intramural sports as well.

INTERSCHOLASTIC SPORTS: Grades 6 through 12

Students who participate in interscholastic sports are required to have a physical examination by a licensed doctor of medicine (MD), doctor of osteopathy (D.O.), or his/her designee (i.e., nurse practitioner) every two years. Documentation of the physical exam must be given to the coach or the school nurse prior to the start of any practices and will be filed as part of the student health record. In addition, the student and parent or guardian shall complete and sign the health questionnaire before the start of each sport.
