Insurance Eligibility

Employees should refer to their applicable master agreement or employment contract for specific eligibility information.   If the information found on this site conflicts with the master agreement/employment contract, the master agreement/employment contract shall take precedence.

In addition to master agreement/contact requirements, eligibility for insurance coverage is also subject to carrier limitations.   Below is a summary of our insurance carriers’ eligibility requirements:

  • BC/BS Medical – Minimum of 17.5 hours/week (0.50 FTE)
  • Northeast Delta Dental – Minimum of 17.5 hours/week (0.50 FTE)
  • UNUM Life Insurance – Minimum of 30 hours/wk (.86 FTE)

Employees Not Eligible for Health Insurance

The Vermont Department of Labor requires us to request a completed Declaration of Health Care Coverage form annually from all employees who are not enrolled in our group health plan.  These completed forms must be maintained on file for a minimum of three years.   Forms must be completed and submitted annually by October 1.  Please click here for a copy of the Declaration of Health Care Coverage form.  Please click here for a copy of the instructions that will assist you in completing this form.
