Welcome to the EHS/CTE School Health page.  We have licensed school nurse/RN staff full time in the office Monday through Friday 7:45 am to 2:50 pm while school is in session.  Please find links to information and forms below.

All students enrolled at EHS should have a record of immunizations for school entry or a signed exemption. Please note that incoming students should now have documentation of two varicella (chicken pox) vaccines or documentation of having the disease.  See below link for Vermont School Entry Immunization requirements.

Yearly parents are asked to complete a Health Information form that will provide necessary information needed by the school nurses, including permission for over the counter pain medication.  Forms are sent home over the summer (see link below).

Students who want to participate in sport activities need a sport physical every two years prior to the start of the season (see link below).

All information is kept in the student’s school health record located in the Health Office. Your student’s medical provider can also fax forms to the health office at 802-879-5503.
