Past Student Transcripts
A transcript request must be made in writing by email, fax or letter.

(put “transcript request” in subject line – very important)
The email address that the request comes from must include the student’s name.

You must include:
- Your full name including maiden name if applicable.
- Date of Birth
- Year of Graduation or when you last attended
- Full name and address of where transcript should be sent.
- A current phone number to contact you if there are any questions.
Transcript will be mailed out within 10 business days from when the request is received.
