Successful Transitions
is a key element in which the EHS Employment Program takes great pride. To ensure that seniors make a successful transition to a job, the staff works closely with adult service agencies. This collaboration facilitates the necessary components that translate into independence. Below is a timeline for school teams and adult service providers follow:
SCHOOL TEAM
- Meet with parents and students to explain transition process
- MAPS to address the needs/desires of student and family
- Develop and revise Transition Plan prior to age 16 to address Training, Education, Employment, and Independent Living Skills
- Discuss guardianship* by age 17
SCHOOL & ADULT/COMMUNITY SERVICE
- Discuss “entitlement” vs. “eligibility”
- Review/revise all areas of the Transition Plan annually
- Explore funding sources, benefits, eligibility and services: Supplemental Security Income (SSI)*, Medicaid Waiver, VocRehab Vermont, Howard Center, supported employment services
- IEP/Transition team comprised of student, family, school and adult services personnel
- Pursue employer-paid job before graduation and/or enroll in post-secondary education
- Smooth transfer of employment/education services from school to adult services
ADULT SERVICES
- Address all areas of transition: Training, Post-Secondary Education, Employment, & Independent Living Skills
- Incorporate Transition Plan into Individual Support Agreement (ISA) & Individual Plan for Employment (IPE)
- Evaluate services regularly
