A transcript request must be made in writing by email, fax or letter.

You must include:

  1. Your full name including maiden name if applicable.
  2. Date of Birth
  3. Year of Graduation or when you last attended
  4. Full name and address of where transcript should be sent.
  5. A current phone number to contact you if there are any questions.

Transcript will be mailed out within 10 business days from when the request is received.
